Topi-Click Testosterone Cream: How Many mg Are In Each Click?

A Topi-Click dispenser meters a fixed volume per click, so the mg you get depends on your cream's concentration. Here are the two formulas, worked examples for both dispenser sizes, and a full reference table.

A Topi-Click releases the same metered volume every click: 0.25 mL on the standard dispenser, 0.5 mL on the larger one. So mg per click = your cream's concentration (mg/mL) × mL per click, and clicks per dose = prescribed mg ÷ mg-per-click. A 200 mg/mL cream in a standard 0.25 mL Topi-Click is 50 mg per click. Your dose is set by your prescriber; this is the arithmetic that turns it into clicks.

What a “click” actually is

A Topi-Click is a metered-dose dispenser: a threaded plunger that advances a fixed distance each time you twist it one notch, pushing out the same volume of cream every click. It exists so you can dose a topical the way you'd dose an injection, by a countable number of increments instead of eyeballing a smear on your fingertip.

The key detail is what a click meters: volume, not milligrams. The standard Topi-Click delivers 0.25 mL per click. The larger (and older) dispenser delivers 0.5 mL per click. That number is fixed by the hardware and has nothing to do with what's inside: it's the same whether the barrel holds a strong compounded testosterone cream or plain moisturizer.

The part every product page skips

Because a click meters a fixed volume, the milligrams in that click depend entirely on your cream's concentration. Two people with the exact same 0.25 mL Topi-Click can get completely different doses per click if their creams are compounded at different strengths — one at 100 mg/mL gets 25 mg a click, one at 200 mg/mL gets 50 mg a click. There is no universal “mg per click” number to look up. It's always concentration × click volume, computed for your specific cream.

The two formulas

Everything in this post is these two lines, applied to different numbers:

mg per click = concentration (mg/mL) × mL per click

clicks per dose = prescribed dose (mg) ÷ mg per click

The first turns the dispenser's fixed volume into a milligram amount using your cream's strength. The second turns the dose your prescriber wrote into a countable number of clicks. Do them in that order and the arithmetic never gets more complicated than this.

Worked example: turning a dose into clicks

Say you have a 200 mg/mL compounded testosterone cream in a standard 0.25 mL Topi-Click, and your prescription is 100 mg per day.

  • mg per click = 200 mg/mL × 0.25 mL = 50 mg
  • clicks = 100 mg ÷ 50 mg = 2 clicks

Now a weaker cream. A 100 mg/mL cream in the same 0.25 mL dispenser, with a 50 mg dose:

  • mg per click = 100 mg/mL × 0.25 mL = 25 mg
  • clicks = 50 mg ÷ 25 mg = 2 clicks

Same number of clicks, but a different dose and a different cream — which is exactly why you can't borrow a click count from someone else. The clicks only line up when the concentration and the dispenser match.

Why the 0.5 mL dispenser changes the answer

This is where most of the mistakes happen. Take that same 200 mg/mL cream and the same 100 mg dose, but put it in the larger 0.5 mL dispenser:

  • mg per click = 200 mg/mL × 0.5 mL = 100 mg
  • clicks = 100 mg ÷ 100 mg = 1 click

One click instead of two: same cream, same dose. Doubling the click volume doubles the mg per click, so it halves the clicks. If you read “2 clicks” on a forum written for the standard dispenser and applied it to a 0.5 mL one, you'd take twice the intended dose. Confirming which dispenser you have is not a detail. It's the whole game.

Confirm the unit on your label first

Compounded cream strength gets written a few different ways, and they are not interchangeable until you convert them. You might see mg/mL, mg/g, or a percentage. A percentage is grams of drug per 100 grams of cream, so 20% = 200 mg/g. Because compounded cream is close to 1 g/mL in density, mg/g and mg/mL come out nearly the same for cream, so a 20% cream behaves like roughly 200 mg/mL. Keep the math in mg/mL and, if your label reads in percent, multiply by 10 to get there (percent × 10 ≈ mg/mL). Always read the exact number and unit off your prescription label before doing any of this.

Frequently asked questions

How many mg is one click of Topi-Click?

It depends entirely on your cream's concentration, because the dispenser meters a fixed volume, not a fixed dose. A click on the standard Topi-Click is 0.25 mL of cream; the larger dispenser is 0.5 mL. Multiply that volume by your concentration in mg/mL to get the mg. A 200 mg/mL cream in a standard 0.25 mL Topi-Click is 200 × 0.25 = 50 mg per click; the same cream in a 0.5 mL dispenser is 100 mg per click.

How do you convert a testosterone cream dose to clicks?

Two steps. First find mg per click: concentration (mg/mL) × mL per click. Then divide your prescribed dose by that number: clicks = prescribed mg ÷ mg-per-click. A 100 mg dose of a 200 mg/mL cream in a standard 0.25 mL dispenser is 100 ÷ 50 = 2 clicks. The dose itself comes from your prescriber. This is only the arithmetic that turns it into clicks.

What's the difference between the 0.25 mL and 0.5 mL Topi-Click?

Only the volume each click releases. The standard dispenser meters 0.25 mL per click; the larger (and older) one meters 0.5 mL. For the same cream, the 0.5 mL dispenser delivers double the mg per click, so you need half as many clicks for the same dose. This is the single most common place people go wrong: copying a click count written for one dispenser and using it on the other doubles or halves the actual dose.

Is testosterone cream dosed in mg or mL?

Both, at different stages. Your prescription sets a dose in mg (the amount of testosterone). The dispenser delivers cream in mL (a fixed metered volume per click). The concentration in mg/mL is the bridge between them: it lets you convert the mL a click releases into the mg it contains, and your mg dose into a number of clicks.

Is a cream concentration the same as a percentage, like 20%?

They describe the same thing in different units, and you have to convert before the click math works. A percentage is grams of drug per 100 grams of cream: 20% is 20 g per 100 g, which is 200 mg/g. Because compounded cream is close to 1 g/mL in density, mg/g and mg/mL come out nearly the same for cream, so a 20% cream behaves like roughly 200 mg/mL. A quick rule: percent × 10 ≈ mg/mL.
