How to Track Your TRT Bloodwork: Markers, Timing & Trends

Which markers matter, when to draw your labs, and why the trend over time beats a single result.

I'd known my testosterone was low for about a year, from an earlier blood test, but going through a clinic was going to cost me almost $200 a month, and I couldn't afford that (see how clinic pricing compares). I was already cutting weight on retatrutide and HGH to hold onto muscle and recovery, and it wasn't quite enough, so the HGH panel I ran doubled as a deliberate baseline before starting TRT.

To track TRT bloodwork, test the same core markers on a consistent schedule and always draw at the same point in your dosing interval, usually trough, right before your next dose. Log every result so you are reading the trend over time, not a single snapshot. The markers that matter most: total and free testosterone, estradiol, hematocrit, and lipids.

Which blood markers matter most on TRT?

Your clinic decides which panel to run and what your target ranges are. This is about knowing what each marker is telling you when the results come back:

  • Total testosterone: the headline number: how much testosterone is circulating in total.
  • Free testosterone: the fraction not bound to proteins, and therefore available to your tissues.
  • Estradiol (E2): testosterone converts into estrogen; tracking E2 often explains how you feel more than testosterone alone.
  • Hematocrit & hemoglobin: TRT can thicken your blood over time. This is the safety marker your physician watches most closely.
  • SHBG: sex hormone binding globulin, which shapes how much of your testosterone stays free versus bound.
  • Lipids: your cholesterol panel can shift on hormone therapy, so it is worth trending alongside the rest.
  • PSA: a prostate marker your physician monitors as a standard part of TRT follow-up.

When should you get bloodwork done — peak or trough?

Timing changes the number. Right after a dose your level peaks; right before the next one it bottoms out at trough. Most clinics draw at trough because it shows your baseline. The single most important rule for tracking: draw at the same point in your dosing interval every time, or you are comparing numbers that were never meant to line up.

When I first started TRT, I drew my labs the morning after an injection — barely a day out, up near the peak. The number came back through the roof, and my stomach dropped: on a standard, low dose, my testosterone had no business reading that high. My doctor saw the same panel and was alarmed too. So I went digging, and it turned out the timing was the whole story — I'd caught myself at peak, not baseline. I explained it to my doctor, they agreed, and we just re-tested a couple of weeks later at a sensible time. Nothing was wrong. It was a peak reading pretending to be my trough. Ever since, I draw right before my next injection, so the number actually means something.

Why track the trend instead of a single result?

One lab result is a snapshot with noise in it: hydration, timing, and normal lab-to-lab variance all move the number. The trend across several draws is the signal. On TRT your levels rise and fall between doses (how much depends on your injection frequency), so a single value with no context (when in the dosing interval you drew, what protocol you were running) can easily mislead. Recording the protocol, the timing, and the result together is what makes bloodwork interpretable.

A trough draw is taken at the lowest point of your dosing interval, right before your next dose. It shows your baseline level rather than a post-injection peak, which is why most clinics use it as the reference point.

Should I test at peak or trough?

Whichever your clinic prefers, the important thing is to be consistent. If you draw at trough one time and mid-cycle the next, the two numbers are not comparable. Pick one point in your dosing interval and stick to it. Trough is the most common choice.

Sources, and one trap

  • The two estradiol assays are not interchangeable, and this is the mistake that wastes the most time. The standard immunoassay and the sensitive LC/MS assay read differently at male-range levels, so a number from one cannot be compared against a number from the other. If you draw sensitive one time and standard the next, the change between them is not a trend, it is two different rulers. Pick one assay and stay on it.
  • Why hematocrit leads the safety list. Erythrocytosis is a labeled adverse reaction on testosterone products, and monitoring haematocrit appears in the labeling itself rather than only in guidance. Depo-Testosterone label.
  • Draw timing matters more than most panels admit. Where you are in the injection cycle changes the number, which is why a result without a recorded draw time relative to the last dose is hard to interpret at all, and why logging the two together is the whole point.
  • What is not claimed: a target number. Reference ranges vary by lab and assay, and where any individual should sit inside one is a clinical judgement, not something a blog post can hand you.
