In men on testosterone it is the hormone testosterone aromatises into, which makes it the single most-watched lab on TRT. As a therapy it comes in several routes that behave very differently: topical cream or gel, transdermal patch, oral tablet, and injectable ester. The most important practical fact about estradiol is not about dosing at all — it is that the two common lab assays are not interchangeable, and comparing a result from one against the other produces a change that never happened. Why estradiol matters even if you never take it

Testosterone converts to estradiol through the aromatase enzyme, so anyone on testosterone therapy is also, indirectly, running estradiol. That is not a side effect to be eliminated — estradiol has necessary roles in bone density, lipid profile, mood, cognition, joint comfort and sexual function in men as well as women.

This is why the reflex to crush estradiol with an aromatase inhibitor causes so many problems. Driving estradiol too low produces its own recognisable syndrome — joint pain, low libido, low mood, poor sleep — that is often mistaken for the dose being too low and "treated" by pushing testosterone higher, which makes it worse. Low estradiol is a real and common iatrogenic problem, not a theoretical one.

The useful framing is that estradiol has a range, not a direction. Too high and too low both cause symptoms, and the symptoms overlap enough that guessing without a lab is unreliable.

The two-assay problem — the thing most people get wrong

Estradiol is measured two different ways, and the results are not comparable.

The immunoassay is the cheaper, more widely available default on a standard panel. It was designed for female reference ranges and is well documented to be less accurate at the lower concentrations typical in men, where cross-reactivity with other steroids can inflate the result.

The sensitive assay — LC/MS or LC-MS/MS, sometimes labelled "estradiol, sensitive" or "ultrasensitive" — separates the molecule by mass before measuring it and is the more accurate method at male-range concentrations.

The practical consequence is blunt: do not compare an immunoassay result against an LC/MS result and call the difference a change. A perfectly stable person can appear to have swung dramatically simply because the lab ran a different method. If you are trending estradiol over time, the only trend that means anything is one built from a single consistent assay, and the assay type belongs in your log alongside the number.

How the routes differ

Topical cream or gel delivers estradiol through the skin, avoiding the liver's first-pass metabolism. Absorption varies with application site, skin condition and surface area, which makes it less predictable dose-for-dose than an injection but easy to adjust in small increments. Transfer to other people through skin contact is a genuine consideration with any topical hormone.

Transdermal patch is the steadiest of the routes, releasing at a fixed rate over days, and is often preferred where stability matters most.

Oral estradiol passes through the liver first, which is the basis of the higher clotting-factor and VTE concern associated with the oral route compared with transdermal or injectable.

What to track

Serum estradiol on a consistent assay, at a consistent point in your cycle or dosing interval. Both of those consistencies matter as much as the number.

On testosterone therapy, estradiol is read alongside total and free testosterone rather than in isolation — the ratio and the symptom picture together are more informative than either number alone. On feminizing HRT, protocols commonly target mid-normal premenopausal levels, checked at a consistent point in the injection cycle.
